Advance Practice Nurse/Physician Assistant
Froedtert & The Medical College of Wisconsin employ a large number of Advance Practice Nurses (Nurse Practitioner, Clinical Nurse Specialist, Nurse Midwife, and Nurse Anesthetist) and Certified Physician Assistants (PA-Cs) within southeastern Wisconsin. Through The Medical College of Wisconsin's adult patient practice, there are a number of specialty patient opportunities, as well as limited opportunities for internal medicine, primary care and family practice. As an academic medical center, we are committed to teaching the next generation of health care practitioners, but recognize that there are a limited number of student slots available.

Clinical Requests/InformationA separate Preceptorship Request Form must be completed for each student that a placement is requested for and must be completed for each semester (if requesting a placement for more than one semester). The form must be completed and returned by the following dates.
April 15 for Fall placements (July 15 for CNS students)
October 15 for Spring placements (November 15 for CNS students)
April 15 for Summer CNS student placements

General Information/FormsThe following information must be reviewed by the student prior to the start of the clinical:
Information for ObserversAn observer is an individual who is 18 years of age or older who is allowed to observe patient care activities at Froedtert Hospital and Clinics. An observer must meet the established requirements identified in the Observer Policy. See the policy and attachments for specific information. The requestor must have an identified staff member that has agreed to the observe/shadow experience and also manager/supervisor approval in the observation area. The forms are kept in the area of the observation. See below for the Froedtert policy (including the confidentiality policy and forms). Anyone visiting an MCW department needs to comply with the MCW policy also. For specific information on Observers in the Operating Room (request form must be completed at least 48 hours in advance and submitted to OR Educator) and Trauma Room, see the attachments.
Computer Training Information/Forms
Other InformationAn affiliation agreement and program memorandum are required for each student clinical experience. These blank documents are available for reviewing student/instructor contract information only and are not meant to be completed by individual instructors or students. Contact Jane Hendricks to check on the status of an agreement between Froedtert and school.
EvaluationsStudents should complete an evaluation of their clinical experience at Froedtert Hospital at the end of the clinical. Please use this link to complete the evaluation. Complete the evaluation.
