Staff pharmacists are important members of the academic team, teaching medical residents, pharmacy students and pharmacy residents. Staff pharmacists also participate in many hospital pharmaceutical studies. Epic Rx Pharmacy Since 2004, the inpatient pharmacy has used the Epic Rx pharmacy information system, providing medication order entry and monitoring tools to facilitate pharmaceutical care throughout the hospital. Epic Rx is also integrated with Froedtert outpatient clinics. Epic Rx works with the Epic electronic medication administration record (eMAR) system to maintain records of all inpatient medications. Froedtert nurses use Epic eMAR for medication documentation and always have up-to-date information on their patients’ medications. All inpatient medication orders are electronically transmitted via Epic Rx to the Central Pharmacy for filling and distribution.
Decentralized Pharmacy Teams (Inpatient Pharmacists)Pharmacists are an important part of specialized inpatient care teams at Froedtert Hospital, providing comprehensive clinical services and patient education throughout the hospital. Pharmacists on inpatient teams participate in daily multidisciplinary rounds, verify physicians’ orders, provide therapeutic drug monitoring, conduct patient medication histories and perform medication reconciliation. Decentralized pharmacist assistants perform order entry, allowing more time for pharmacists to conduct these patient care activities.
Decentralized pharmacy teams provide inpatient services throughout the hospital, including in Blood and Marrow Transplant, Neuro Intensive Care, Surgical Intensive Care, Medical Intensive Care, Cardiac Intensive Care, Solid Organ Transplant, Adult Internal Medicine, Orthopaedics, the Emergency Department and the Operating Room.
Central PharmacyThe Central Pharmacy, staffed 24 hours a day, uses advanced technology to fill inpatient medication orders efficiently and safely. The Central Pharmacy uses the latest prescription fulfillment technology, which decreases the likelihood of medication errors with barcode scan verification and dispensing. This technology ensures the right drug is prescribed for the right patient at the right time. The system also manages the drug inventory.
Pharmacists in Central Pharmacy are responsible for ensuring the proper preparation of intravenous medications and verification of medication selection. They also provide order entry services for the Emergency Department, Birth Center and outpatient Infusion Clinic, and answer drug-related questions from staff throughout the hospital.
Outpatient and Ambulatory Pharmacy Pharmacists staff three retail pharmacies at Froedtert Hospital. The pharmacies are located in the east and west ends of the hospital and in the new Froedtert & the Medical College of Wisconsin Clinical Cancer Center (scheduled to open in May 2008). The outpatient pharmacies serve Froedtert inpatients and ambulatory patients who are seen in a Froedtert or Medical College of Wisconsin clinic.
Pharmacists also evaluate patient medication regimens, monitor lab values and provide patient counseling in various Froedtert Hospital outpatient clinics, including the Infectious Disease Clinic, Anticoagulation Clinic, Hypertension Clinic and Diabetes Clinic. Pharmacists in the Hematology/Oncology Clinic evaluate medication regimens and provide safe preparation and administration of chemotherapy agents.
ASHP Best Practices AwardIn December 2007, the Froedtert Hospital Pharmacy Department was honored by the American Society of Health Systems Pharmacists (ASHP) at its Midyear Clinical Meeting and Exhibition. ASHP selected Froedtert pharmacists to receive the ninth annual ASHP Best Practices Award in Health-System Pharmacy for the program titled “Improving Patient Safety through Implementation of a Pharmacy-Conducted Admission Medication History and Multidisciplinary Discharge Medication Reconciliation Process.” This award program is a national competition that recognizes innovation and outstanding leadership in health-system pharmacy. Froedtert pharmacists were one of six recipients selected for the award in 2007 from among 60 applicants.
