   • Approximately every fourth weekend and one weekly shift (total of five shifts/month)

6. Licensure
   
   
   • Licensed as a pharmacist in the state of Wisconsin
   
   
7. Other
   
   
   • Minimum eight hours/day (Monday - Friday)
   • Assist and support department in additional ways as needed
   • Complete assignments and evaluations on a timely basis
   • ACLS certification (classes provided)
   • Coordinate Pharmacy Week activities within the hospital
