PGY1 Residency Project
Effects of a Pharmacist Managed Clinical Service for the Utilization and Management of Direct Thrombin Inhibitors (DTIs) for the Treatment of Heparin-Induced Thrombocytopenia (HIT)
Due to the complex diagnosis of HIT and lack of standardized utilization of IV DTIs, a new Froedtert Hospital pharmacist managed guideline was initiated in June 2011. I will complete a pre/post guideline analysis to justify the role of the new guideline as a tool for the clinical assessment and treatment HIT. The current guideline includes IV DTIs argatroban and lepirudin.  Another IV DTI indicated for HIT, bivalirudin, may prove to be more cost effective and act as a treatment option for patients with a contraindication to argatroban and/or lepirudin. Therefore, I will evaluate the addition of bivalirudin to the Froedtert guideline.
