Optimizing Medication Reconciliation Communication between Hospital and Nursing Home upon Discharge
The purpose of this study is to optimize communication between hospital and nursing home upon discharge, as it pertains to medication reconciliation. The project involves evaluating existing inefficiencies in order to reduce potential medication errors.
Why did you choose to pursue a residency?
A residency provides unique opportunities that I may not otherwise experience in a traditional pharmacy role. I wanted to be exposed to a variety of interests in order to find the right career path for me, and become a more confident and skillful clinician to better serve my patients.
What attracted you to Froedtert’s pharmacy residency program?
I had the pleasure of having one of my fourth-year rotations at Froedtert which allowed me to get to know the organization and its employees. I really enjoyed my experience and was impressed with pharmacists’ presence and expanding roles in outpatient settings. What attracted me to this residency program was the focus in ambulatory care within a large academic health system, staffing in the outpatient pharmacies and the wide variety of experiences and opportunities offered.
