Clinical Requests/InformationA separate Request for Clinical Group Placement form should be completed for each clinical group you are requesting placement for. The form needs to be completed and returned by the date on the form for spring, summer, fall, and precepted placements. For review of student/instructor contract information, a sample Froedtert Hospital affiliation agreement and program memorandum are included below.
General Information/FormsThe Competencies/Skill Assimilations that are listed will be performed during classroom orientation by all new nursing instructors.
Instructor InformationThe forms and information included are for new and current nursing instructors and coordinators. This includes clinical group instructors and precepted instructors/coordinators.
Computer Training Information/FormsInstructors and students are required to attend a 4.5 hour Epic Inpatient Computer documentation training class that includes an online assessment during the first 15 minutes of class that will require an 85 percent to pass. Students will be sent home from class and will need to reschedule the class if the assessment is not passed. Maps are located below.
Documentation and medication administration on the unit may not be done until the class is completed.
Online learning modules (approximately 10 to 20 minutes each, 2 to 3 hours total time available through the Froedtert Hospital Learning Center) must be completed prior to attending the computer documentation class. Instructions for remote access to Froedtert Hospital's Learning Center for completion of the required online training modules will be sent to the schools/instructors after successful submission of the Computer Access Spreadsheet.
The Computer Access Spreadsheet must be completed and submitted (in Microsoft Excel 2003 or prior version) to Jane Hendricks, Education Coordinator, at jhendric@fmlh.edu, at least 14 business days prior to the class. The spreadsheet must include both instructors and students. If you have precepted students, they will be scheduled into existing computer classes.
The computer access spreadsheet template must be completed each semester for all instructors and students (even if they have already taken the Epic Inpatient Computer documentation training class) in order to get reinstated into the Froedtert computer system.
If you have not already made arrangements for a computer class, contact Jane Hendricks, Education Coordinator, at 414-805-5392 or via email at jhendric@fmlh.edu. Any changes in students that will affect attendance in computer classes should also be sent to the Education Coordinator.
The Security Questions form and Confidentiality Agreement should be faxed directly to IT using the Computer Fax cover sheet prior to the class.
Students here for observational experiences only are not required to participate in computer training.
Students in outpatient/day surgery/clinic, and OR/PACU areas will also require EPIC classes. Contact Jane Hendricks, Education Coordinator, at 414-805-5392 or via email at jhendric@fmlh.edu, for access request forms and classes, etc.
Staff Development will no longer be able to facilitate computer/program access and password changes in the classroom. After a student or instructor has attended and completed their required computer training they will need to contact the Froedtert and Community Health Service Desk at 414-805-2101 from the nursing unit. It will be the responsibility of the nursing instructor to facilitate this for their clinical groups. All students and instructors will also be given a handout after computer class outlining this process. The Service Analyst will assist them with their password changes over the phone.

EvaluationsAll instructors/coordinators and students should complete an evaluation of their clinical experience at Froedtert Hospital at the end of the semester or clinical. Instructors/coordinators will be sent information about completing the evaluations each semester. This information should be forwarded to the students in their clinical group(s) and/or their precepted students. The evaluation should be completed using the online form. The staff at Froedtert will also be completing an evaluation. Your responses and your student responses will be collated and shared with the directors/managers, and staff on the unit. The staff responses will also be collated and sent to you and the schools.
Information for ObserversAn observer is an individual who is 18 years of age or older who is allowed to observe patient care activities at Froedtert Hospital and Clinics. An observer must meet the established requirements identified in the Observer Policy. See the policy and attachments for specific information.
