Froedtert Health Benefit Offerings

You will need your user name and password that were included in the e-mail your received from the Benefits Department.

Wellness Works – Current Staff

The Froedtert Health four step HRA process for current staff will begin with the online “Personal Wellness Profile” on May 1, 2014.

  • All staff is invited to participate in the HRA.
  • Staff enrolled in the medical plan or staff that plan to enroll in the medical plan who complete the four step HRA process prior to September 12, 2014, will be eligible for the 2015 medical plan discount of $15.00 per pay period/up to $360.00 annually.
  • The four step HRA process includes:
    • The online HRA Personal Wellness Profile (PWP)
    • Logging 50 “Active Participation” credits on the Health Activity Tracker
    • Biometric testing
    • Coaching as indicated by overall wellness score ≤59.

Current staff may begin completing the online Personal Wellness Profile May 1, 2014. See Scout/Wellness Works/HRA for additional information and a listing of all biometric clinics.

Access from home: The Wellness Works Wellness Portal requires Internet 7.0, 8.0 or Fire Fox 2.0 or higher to run.

Current Staff Wellness Works Portal

Wellness Works Portal Log-In Instructions: Beginning May 1, 2014, all passwords have been reset to the following values.

  • Username: Staff ID number
  • Password: The word "WELL" (in capital letters) followed by the last four digits of your social security number
  • You will be instructed to change your password after the initial login.
  • Click "Assess Your Health Now" found in the upper right corner of the portal homepage to begin.
  • Complete the online “Personal Wellness Profile.

Wellness Works - New Hire and Status Change Staff – 2014 Health Risk Appraisal Information

The instructions for completing the online Health Risk Appraisal are for newly hired staff or a staff member with a qualifying status change.

  • Staff hired June 1, 2014, or later will only need to complete the online Personal Wellness Profile to be eligible for the medical plan discount ($15/pay period) for the remainder of 2014 and 2015.
  • Staff hired prior to June 1, 2014, will complete all 4 steps of the HRA process to be eligible for the 2015 Medical Plan Discount.

Access from home: The Wellness Works Wellness Portal requires Internet 7.0, 8.0 or Fire Fox 2.0 or higher to run.

Wellness Works Instructions

  1. Click the New Hire Wellness Works Portal.
  2. At the log-in page, click, “I don’t have an account: Sign Up.”
  3. Complete all fields with a red asterisk.
  4. At portal homepage, click "Assess Your Health Now" found in the upper right corner to begin the "Personal Wellness Profile."
  5. Complete your Personal Wellness Profile.

Lawson Self-Service

Access the Lawson Self-Service by entering your network log-in name and password. Lawson Self-Service provides staff with a point of access to their own personal, benefit and payroll data via a secure web interface, such as:

  • Current Tax Withholding
  • Pay Check Detail (up to last 4 years of payments)
  • Year-to-Date Hours, Dollars and Deductions
  • Payment Modeling Feature
  • Current Benefit Enrollment
  • Benefit Provider Directories
