Medical College of Wisconsin Selected
for National Diabetes Care Program


The Medical College of Wisconsin is one of 22 medical schools nationwide and the only one in Wisconsin chosen by the Association of American Medical Colleges to participate in a program designed to improve care for patients with diabetes.

At Froedtert & Medical College of Wisconsin, a team, including a primary care doctor, nurse, medical assistant, and diabetes educator, helps patients identify personal health goals and monitors their progress. Patients comment that until now, no one has helped them so thoroughly understand what they can do to stay healthy.

"Our focus is on improving the care of diabetic patients and reducing hospitalizations and complications," says Jaishree Hariharan, MD, Medical College of Wisconsin internist and overall leader for the national project. "This program also enables us to teach the next generation of physicians the most effective methods to manage chronic diseases."
