Patients who do not bring advance directives with them to Froedtert & Medical College of Wisconsin can get the necessary forms (and assistance filling them out) from a social worker or a chaplain. To ensure the hospital has the most current version, you must provide advance directives for each hospital stay. Do advance directives “lock you in” to certain choices?
