Community Memorial Medical Commons

Serving Menomonee Falls and Surrounding Communities



Community Memorial Medical Commons offers leading medical experts and comprehensive care for adults and children.

The services at the Medical Commons are offered through a partnership between Froedtert & The Medical College and Froedtert Health Community Memorial Hospital that results in coordinated care for our patients.

When needed, our physicians refer patients to other medical services within the Medical Commons. Hospital care, if required, will be provided at one of our three affiliated hospitals: Community Memorial Hospital in Menomonee Falls, Froedtert & The Medical College of Wisconsin in Wauwatosa, or St. Joseph's Hospital in West Bend.

As a patient, you'll also benefit from:

  • Common scheduling system. With one phone call, you can schedule appointments with any physician or service at the Medical Commons.
  • Electronic medical record to enhance continuity of care. This is especially important for patients who receive care from many physicians and services. All patient information is documented in a single record, allowing all caregivers to view every consultation, treatment and test result.
  • Digital radiology. Any physician at Community Memorial Medical Commons is able to view imaging results or the status of an imaging test at any time.
  • Same-day access to on-site lab services provided by Dynacare Laboratories and diagnostic imaging provided by the Center for Diagnostic Imaging.
  • Convenient access from Hwy. 45 and ample free parking.

Primary Care Clinic
Building A, Suite 100
Staffed by Medical College of Wisconsin primary care physicians in internal medicine, obstetrics/gynecology and internal medicine/pediatrics. Hours:
Monday-Thursday: 8 a.m.-7p.m.
Friday: 7 a.m.-5 p.m.
Walk-in Clinic
Building A, Suite 102
Designed for urgent medical needs of children and adults. No appointment is needed. Hours are Monday through Friday, 12:30 to 8 p.m., and weekends, 8:30 am to 4 p.m.

Cardiothoracic Surgery Clinic
Building A, Suite 201
Provides consultations and surgical intervention for cardiac patients. Our physicians include Medical College of Wisconsin physicians Alfred Nicolosi, MD, and Charles Reuben, MD.

Cardiology Clinic
Building A, Suite 201
Offers diagnosis and treatment of cardiovascular illnesses and diseases by Medical College of Wisconsin cardiologists Alexei Agapitov, MD, and David L. Groden, MD, FACC, FAHA, FASE; and electrophysiologist Jason Rubenstein, MD, and Dalip Singh, MD.

Cardiology Testing
Cardiology diagnostic services include echocardiography, nuclear cardiology studies and stress testing.

  • An echocardiogram, or "echo" as it is sometime called, uses ultrasound technology to take images of your beating heart. An echocardiogram provides valuable information about your heart and how blood flows through it.
  • Nuclear cardiology testing uses radioactive materials (tracers/isotopes) to obtain diagnostic images of the heart and vessels. Testing involves giving a tiny amount of a tracer to a patient. The tracer collects in the heart and gives off gamma rays, which are detected by a gamma camera. Skilled nuclear medicine technologists perform the tests, which are interpreted by experienced Medical College of Wisconsin cardiologists.
  • Stress tests determine how the heart functions under stress. The test can show if the blood supply is reduced in the arteries that supply the heart. Stress tests may be done on a treadmill or with the use of a medication that stimulates the heart to pump faster, simulating exercise.

Electrophysiology Clinic
Offers specialized care in the diagnosing and treatment of diseases related to the electrical activities of the heart by Medical College of Wisconsin electrophysiologists Dalip Singh, MD, and Jason Rubenstein, MD.

Endocrinology Center

    Orthopaedic Surgery Clinic

    Physical Medicine and Rehabilitation Clinic

    Psychiatry and Behavioral Medicine Clinic

    Pulmonary Medicine Clinic

    Specializes in treating patients with diseases of the lungs and respiratory tract. This clinic also includes a full range of pulmonary function testing services for issues such as asthma, lung capacity, chronic obstructive pulmonary disorder (COPD) and heart disease.     Rheumatology Clinic
    Building B, Suite 402
    Focuses on problems involving joints, soft tissues and connective tissues including arthritis, scleroderma and lupus by Medical College of Wisconsin rheumatologist Richard Hariman, MD.

    Senior Health Memory and Mood Disorders Clinic
    Building A, Suite 302
    Provides treatment for the mental health needs and specific syndromes faced by older adults. Geropsychiatrists Joe Goveas, MD, Harold Harsch, MD and Dami Salami, MD, specialize in disorders such as Alzheimer’s dementia, delirium, other conditions that affect memory and the ability to function in daily activities of life, and late-life depression, anxiety and psychotic disorders.

    Sleep Medicine Clinic

    This clinic offers a thorough diagnosis of sleep disorders and provides education, treatment options and rehabilitation. Our expert team includes otolaryngologist B. Tucker Woodson, MD, and pulmonary medicine specialists Theodore Hubley, MD, and Tilottama Majumdar, MD, with access to experts in neurological diseases, reconstructive surgery and psychology.

    SpineCare
    Building B, Suite 402
    An outpatient program that provides individualized, exercise based care for people with musculoskeletal spine disorders. Our transdiciplinary team includes orthopedic and neurosurgeons, physical medicine and rehabilitation physicians, chiropractic physicians, nurses and physical and occupational therapists.

    Urology Clinic
    Building A, Suite 303
    Offers diagnosis, treatment and management of urological diseases. Our physicians are Jeffrey Bejma, MD and R. Corey O’Connor, MD.

    Regional Sleep Disorders Center

    Froedtert Health Community Memorial Hospital’s Regional Sleep Disorders Center provides high quality, compassionate healthcare to people who suffer from sleep disorders. Certified sleep technologists monitor patients with state-of-the-art sleep acquisition equipment. The Sleep Center features six sleep-study bedrooms with additional amenities designed for patient comfort.

    Offers a full range of diagnostic imaging, Computed Tomography (CT), X-ray, ultrasound and digital screening mammography.

    Cardiac and Pulmonary Rehabilitation

    Certified by the American Association of Cardiovascular and Pulmonary Rehabilitation, this program provides monitored exercise and diet/lifestyle education to rehabilitate cardiac patients, and pulmonary rehabilitation to help patients with chronic pulmonary disease.

    Dynacare Laboratory

    A full-service clinical laboratory with convenient hours:
    Monday - Friday 7 a.m.-8 p.m.

    Rehabilitation and Sports Medicine Center

    Froedtert Health Community Memorial Hospital’s Rehabilitation and Sports Medicine Center provides general physical therapy, orthopaedic rehabilitation, sports medicine and athletic training services.

    Worker Assessment and Rehabilitation Center
    Building B, Suite 400
    Offers comprehensive services to treat work injuries and chronic pain by Froedtert Health Community Memorial Hospital. The comprehensive rehabilitation team consists of physical and occupational therapists, vocational rehabilitation counselors and psychologists.
