You will speak to one of our nurse coordinators who will ask you a series of questions to explore your medical history, assist you in acquiring your medical information from your current provider and schedule your appointment with the expert who is best for your condition. We will work with you and your insurance provider to make sure your visits are covered.
Online Requests
Use our convenient online request form. Our nurse coordinators will call you as a follow-up to this online request. You will be provided a special program folder in which you can keep your records and information, as well as take notes during your visits. This includes a follow-up letter which will be presented to you at the last exam.
