Community Memorial Hospital offers a state-of-the-art, 17 bed inpatient rehabilitation unit that focuses on providing the best quality care in order to return our patients to independent living.
Our dedication to high-quality, individualized rehabilitative care has earned us accreditation by the Joint Commission and the Commission on Accreditation of Rehabilitation Facilities (CARF). Our Stroke Specialty Program, a rehabilitation program designed specifically to meet the unique needs of stroke patients, also earned CARF accreditation. Specialized Care for a Range of Issues
We serve patients who are functionally challenged by impairments related to developmental disabilities, disease, and/or injury. We provide specialized care for individuals recovering from:
- Spinal cord injury
- Cardiac disease
- Traumatic brain injury
- Neuromuscular disease (i.e. Multiple Sclerosis)
- Neurological disease (i.e. Guillain-Barre)
- Pulmonary disease
- Complex medical problems
- Orthopedic problems
Rehabilitation Facility Environment
Our state-of-the-art facility is designed to simulate a home environment as much as possible while ensuring comfort and social interaction. Each room is equipped with a private bathroom and shower. Our unit also boasts a private family room, spacious dining and activity room, and private treatment room.
Individualized care begins on day one to ensure a discharge that is as safe, comfortable, and stress-free as possible. Patients can expect at least 3 hours of combined therapy time per day for 5 to 7 days a week until discharge.
Our experienced staff follows a patient-centered philosophy committed to achieving each patient's goal.
Our team takes into consideration and plans for services that will be needed after discharge such as nursing care, outpatient therapies, Meals on Wheels, Adult Day Care Programs, financial assistance, economic needs, housing, and safety concerns.
Each patient participates in weekly interdisciplinary staff meetings where family/significant others are encouraged to attend to discuss progress, concerns, discharge plans.
Community Integration Programs
Patients will also have the opportunity to participate in various community integration programs such as:
- Home assessments
- Home passes where the patient may leave the premises for 4-6 hours when deemed appropriate
