Examples of the Academic-Community Model
Through the Froedtert & the Medical College of Wisconsin Cancer Network, cancer care is provided in the community hospital setting whenever possible. If patients need a specialized treatment, they may connect with Froedtert & the Medical College — and return home for follow-up care. Through this connection, the Cancer Network brings more communities direct access to nationally and internationally known specialists who focus on specific cancers, advanced treatments and technologies, and clinical trials that may provide further options.
Froedtert Health is the first health system in the region to offer a telestroke program, connecting board-certified stroke specialists from the academic medical center with community-based hospital emergency departments.
Working Together for Accountable Care
Froedtert Health is a member of Integrated Health Network of Wisconsin, an aligned group of hospitals, clinics and physicians, working together to provide the right care at the right time and place. This network, representing 28 hospitals and 4,000 physicians in three states, creates a broad base of providers that, through clinical integration, can provide a continuum of care management options.
