Patients who recently visited a Froedtert Health emergency department with any of these diagnoses receive a follow-up call to ensure they are improving and to catch problems early. Nurses review the discharge instructions, address questions, ask whether the patient has a primary doctor for follow-up and review care plans. If a patient reports symptoms, the nurse triages the patient according to clinical protocols to the most appropriate level of care such as office visit, urgent care or emergency.
