Medication Therapy Management Continuum of Care Service Targeting High Risk Patients at Discharge

Hospital readmission rates are a problem plaguing health care systems nationwide. Medication-related problems are one factor contributing to these high readmission rates. One potential strategy for combating medication-related readmissions is the provision of Medication Therapy Management (MTM) services soon after discharge. As the medication experts, pharmacists are in a prime position to improve patient outcomes, help close the gaps in transitions of care, and reduce readmissions through the provision of MTM services focused on patients at high risk for medication-related readmission. The primary objective of this project is to develop and implement a sustainable MTM service targeting high risk patients for follow up soon after discharge at Froedtert Hospital.

Development and Implementation of a Targeted First Dose Medication Teaching Service

Improving patient satisfaction and expanding patient centered care are major areas of focus for institutions nationwide. While it is well-established that providing medication teaching at the point of discharge improves patient outcomes, patients may be overwhelmed with information on the day of discharge. Therefore, one additional strategy for improving patient medication understanding is to provide medication teaching throughout hospitalization. This will also enhance the visibility of the pharmacist as a member of the health care team from the patient perspective. The primary objective of the project is to develop and implement a pharmacy-driven targeted first dose medication teaching service for patients (or their respective caregivers) receiving targeted medications on selected units.

Why did you choose to pursue a residency?

I chose to pursue a residency to continue my professional development, gain more experience in various areas of health system pharmacy, and build clinical skills and knowledge. It is my goal throughout residency to establish a strong foundation for a successful career.

What attracted you to Froedtert’s pharmacy residency program?

The PGY1/2 Health System Pharmacy Administration program at Froedtert Hospital appealed to me for many reasons. I searched for a residency training program at an academic medical center with a Department of Pharmacy Services with a strong reputation for excellence in the provision of innovative, progressive clinical pharmacy services, use of advanced technology, and outstanding pharmacist training programs. One of the key deciding factors for me was the emphasis on the development of a strong clinical background during the PGY-1 year, followed by a more administrative focus during the PGY-2 year. I also sought a large program that would help me to build lasting relationships and a network with my co-residents and preceptors. However, what really left a lasting impression on me throughout the interview process was how welcoming, passionate, and helpful the Froedtert residency program directors, preceptors, and current residents were. It truly is the people that make the residency program so great!

What's your favorite thing to do in Milwaukee?

Living downtown has been the best experience. I enjoy spending time on the lakefront, Brady St, Water St, Bradford Beach, Tosa Village, and shopping at local farmer markets. And of course, Summerfest and the many, many festivals that occur every year in Milwaukee!
