Join therapists from the Froedtert & the Medical College of Wisconsin Neurologic Rehabilitation Program for a six-week exercise course designed for people with Parkinson’s disease.

• Interested participants will be chosen using a lottery system due to high demand. Participation status will be confirmed closer to the class start date.
• To be eligible, you will need to complete a waiver signed by your physician before attending class.
• If you have concerns about your balance or risk of falling, bring a care partner to assist you. If our team members have concerns, they may request that you bring a care partner.

Please fill out the following forms. You will fill out the patient waiver for yourself. You will need to have your doctor sign the appropriate waiver. Bring the signed documents to class.
