What type of requests does the Froedtert & MCW health network consider?

If you have additional questions, or the guidelines do not address your questions, we ask that you submit the form for evaluation.
How do I submit my request?
After you have completed the questionnaire, proceed to the final step and click "Submit" Please review your request carefully before submitting it – if you've failed to answer a required question, the system will remind you. You will receive a confirmation e-mail upon successful submission of the form.
What is the status of my request?
You will receive a letter with the status of your request.
Can my existing request be reconsidered?
All decisions made on a specific request are final, but you can always submit a new request and refer to a previous request date. This is especially encouraged if important information has changed or a significant amount of time has passed since your initial submission.
This system doesn't seem to apply to me. How do I speak to someone directly?
Without understanding your specific needs, it is highly recommended to continue to submit your request through this system. This will be the most efficient way to route your request to the proper representative. Once the request is in the system, we can route it to the appropriate Froedtert & MCW representative who will contact you.
What if I am requesting volunteers or help with a health fair?
There is a portion of the application that speaks specifically to this. Please make sure that you fill out the appropriate box with the specific needs you have.
