The Froedtert Hospital Community Investment Fund Committee is seeking Request for Applications (RFA) from community-based organizations, and nonprofits for innovative projects that improve health outcomes and reduce health disparities in underserved communities.  

Purpose of the RFA

RFA’s should address the Froedtert Hospital Community Health Needs Assessment priority health needs, social drivers of health, and/or promote health education and prevention. 

Please keep in mind that the Froedtert Hospital Community Investment Fund does not provide funding for the following:

  • Requests that benefit an individual
  • Political campaigns and/or elected officials
  • Fundraising initiatives
