Community Health Needs Assessment
Froedtert Health is a member of the Milwaukee Health Care Partnership, a public private consortium dedicated to improving care for underserved populations in Milwaukee County. Through the Partnership, five health systems and the Waukesha County Health Department aligned resources to complete a shared community health needs assessment (CHNA) from June to September 2017. Supported by additional analysis from the Center for Urban Population Health, this robust community-wide CHNA includes findings from a community health survey of over 400 adults, 50 key informant interviews and a secondary source data analysis.

In March 2015, Community Memorial Hospital collaborated on a community health needs assessment with JKV Research, LLC, along with Aurora Health Care, Children’s Hospital of Wisconsin, Columbia St. Mary's Health System, ProHealth Care, Wheaton Franciscan Healthcare and the Waukesha County Public Health Department. The assessment was the compilation of state, county and community health assessments along with interviews from 50 key community informants ranging from school district representatives, public health officials, non-profit health and human service professionals, police departments, free clinics and business professionals throughout Waukesha County.

Community Memorial Hospital's total benefit to the community in fiscal year 2017 was more than $17 million.
Community Outreach Steering Committee Grant
Community Memorial’s commitment includes grants to local not-for-profit organizations that support community health improvement efforts focused on access to primary care, chronic disease management, mental health and prevention and wellness in the hospital’s service area. Community Outreach Health Clinic
The Community Outreach Health Clinic is a community-based, private, non-profit autonomous agency governed by Community Outpatient Services with a mission to:
- Provide quality, non-emergency medical, dental and pharmaceutical care
- Refer clients to appropriate community services
- Encourage a preventive, wellness lifestyle through education and counseling
To uninsured and underinsured persons residing in Community Memorial Hospital’s service area, using volunteer professional staff.
