I feel she was very thorough in her evaluation of my health issues. I think she went above tracking down some information about what I had been hospitalized for a few months prior to my visit with her. I would like to thank her for her soothing courtesy. It gave me confidence in her.

Medical record not available due to computer failure; physician did not read prepared survey document prior to start of appointment - was reading document while conducting exam; tech was not very attentive telling us to follow her to exam room while she was standing behind us; found hallway to exam room very congested and chaotic (lack of confidentiality; physician and tech were not adequately willing to asset patient find bathroom when patient experienced urgent need to use bathroom (patient has significant cognitive decline); physician's final rapport did not appear to be proof read containing many types/misspelling and unclear sentence fragments. In general this experience was NOT up to the standard that we have encounter in other areas of Froadtert & Medical college of Wisconsin.
