Milwaukee, Wis. — Froedtert & MCW health network and CVS Health have formed a new clinical affiliation to help enhance coordination of care for patients.
The affiliation enables CVS Pharmacy locations and the Froedtert & MCW health network to securely integrate certain portions of their electronic health record systems. The arrangement will allow Froedtert & MCW physicians, CVS pharmacists and practitioners to securely exchange information to better ensure patients take prescribed medications and adhere to care plans.
“We’re continually improving how we deliver patient-centered care both inside and outside of our clinics and hospitals,” said Froedtert & MCW Enterprise Chief Medical Officer Jonathon Truwit, MD. “By giving physicians access to a more complete and timely profile of their patients, some of whom are managing chronic conditions such as diabetes, this partnership is another way we are demonstrating a commitment to integrated, quality care.”
Froedtert & MCW health care providers will receive messages and alerts from CVS Pharmacy regarding patients’ medication issues. In addition, CVS MinuteClinics will electronically share patient visit summaries with the patient’s primary care physician if the physician is part of a Froedtert & MCW affiliate organization and with the patient’s consent.
