Froedtert & MCW health network partners with Horizon Home Care and Hospice to provide comfort care to patients who are reaching the end of life while in the hospital and are too fragile to be moved home or to a hospice facility.
Horizon hospice care is available at Community Memorial Hospital and Froedtert Hospital to offer additional support for patients and their families with a focus on comfort, symptom relief and pain management. Hospice care at these locations offers:
- A collaborative model of care: 24/7 support provided by Froedtert & MCW nurses and palliative care experts, partnering with Horizon’s highly trained staff for support and an improved patient/family experience
- Ongoing communication and education for family members about their loved one
- Help for family members coping with grief
- Spiritual support
- Light refreshments and comfort items for family members who may spend long periods of time at the patient’s bedside
At Community Memorial Hospital, the six-bed Garden View unit offers all the benefits of hospice care in a private, peaceful area with views of the hospital’s Healing Garden and a family gathering space.
At Froedtert Hospital, hospice care occurs on the inpatient unit where a patient is receiving care. In addition to the above, hospice enhances the patient’s room with comforting features such as ambient lighting and aromatherapy.
