The Joint Notice of Privacy Practices for Protected Health Information applies to the health care provider entities that are affiliates of Froedtert ThedaCare Health, Inc. and to The Medical College of Wisconsin, Inc. and its affiliates.

The health care provider entities that are affiliates of Froedtert ThedaCare Health, Inc. and that are designated as an Affiliated Covered Entity for purposes of HIPAA include the following entities:
