Notice to Our Patients of a Privacy Incident

Froedtert Health is committed to maintaining the highest level of privacy and security of our patients’ protected health information. Regrettably, this notice is regarding an incident that involves some of that information.

On December 1, 2020, we learned that an employee viewed certain medical records pertaining to patients without a known business purpose. Based on its investigation, Froedtert Health determined that the employee may have viewed some medical records without a known business purpose, which may have included the following types of patient information: demographic information, dates of birth, medical record numbers, and documentation related to the care and services that certain patients received. No Social Security numbers or financial information were included in this incident. The individual is no longer employed by Froedtert Health and no longer has access to any patient information. The incident impacted a limited number of Froedtert Health patients.

We have no evidence to indicate that any patient information was used for any malicious purpose or that it was further used or disclosed. However, out of an abundance of caution, we began mailing letters to affected patients on January 15, 2021, and established a dedicated call center for patients to call with questions. If you believe you were affected but do not receive a letter before February 15, 2021, or would like more information about this incident, please call 1-833-978-2825, Monday through Friday, 8:00 a.m. to 6:00 p.m. Central Time. We also recommend that our patients review any statements that they receive from their healthcare providers. If patients identify any services that they did not receive, patients should immediately contact the provider.

Froedtert Health sincerely regrets that this incident occurred and apologizes for any inconvenience or concern to its patients. All Froedtert Health staff receive comprehensive privacy training and Froedtert Health is evaluating additional measures that may be put into place to prevent similar incidents from occurring in the future.
