Froedtert & MCW health network opens Forest Home Health Center

Milwaukee — Today, the Froedtert & the Medical College of Wisconsin health network opened the Forest Home Health Center, which will also house the new South Side Family Medicine Residency Program. Located at 1316 W. Forest Home Avenue, Suite A, Milwaukee, the center provides primary care for all ages and needs, including kids and older adults, and will improve access to preventive care so health problems may be detected at early stages when they may be easier to treat. The health center provides prenatal care, care after childbirth and care of newborns after hospital discharge. Behavioral health services are also available.

“Our family physicians take the time to get to know you and develop a personalized health care plan,” said Sabrina Hofmeister, DO, residency director, and associate professor at MCW. “We also offer quick and easy access to appointments, including online scheduling options, extended hours to accommodate busy lifestyles, and Spanish-speaking clinicians.”

The South Side Family Medicine Residency Program, co-located with the Forest Home Health Center, is designed to train more family medicine physicians in the Froedtert & MCW health network. It will provide quality medical education in an urban community setting, advance health equity among Milwaukee’s predominately Hispanic/Latinx community, and increase community engagement and federally funded research efforts. Aligned with the health network's overall growth strategy, this new health center and residency program will support better access to quality health care in local communities. Forest Home Health Center will provide culturally and linguistically equitable care, which aligns with our EDI commitments and efforts.
