Community Health Needs Assessment

Every three years, Froedtert ThedaCare aligns resources with JKV Research, LLC, other area health systems and local health departments to participate in a shared data collection process called the Community Health Needs Assessment (CHNA). The CHNAs serve as the foundation for each hospital’s implementation strategies to improve health outcomes and reduce disparities. Learn more about CHNAs and implementation strategies.

Milwaukee Health Care Partnership

Froedtert ThedaCare is a member of the Milwaukee Health Care Partnership (MHCP). Established in 2007, the MHCP is a public/private consortium dedicated to improving health care for low-income populations in Milwaukee County with the aim of improving health outcomes, advising health impact and lowering the total cost of care.

With a focus on expanding health care coverage, access, care coordination and community health, the partnership brings together health care providers, government agencies and community organizations to collaboratively improve the health of underserved people in Milwaukee County.
