Routine clinic appointments are Monday, Wednesday and Friday afternoons between 12:30 and 4:30 p.m. The clinic doctors and clinicians are rounding on patients in the hospital during the morning or seeing patients receiving infusion services. Same-day urgent appointments are made on an as needed basis. Please call 414-805-2220 for an appointment.

When you call to make an appointment, you need to have some basic information ready. The below is a roadmap for when you call. When you call, the person answering the phone will transfer you to the sickle cell eisease scheduler.

What to Bring to Your Appointment

  • Insurance card
  • Identification
  • List of medications
  • Allergies
  • Any papers or records from your previous doctors
  • Any questions you want to ask the doctor

Transportation to Your Appointment

If you have Medicaid or a Medicaid HMO (Forward Health Card), you qualify for rides to medical appointments through your insurance. If a family member or friend is able to bring you, you may also qualify to be paid for mileage. To schedule transportation, you will need to call at least 48 hours before your appointment. It’s best to call Medical Transportation Management (MTM) as soon as you know about your appointment.
